Abstract


Main of our study was to report on the age at time of surgery and the therapeutic features among children who underwent surgery for hydrocele in Dakar. Materials and methods: This was a retrospective study conducted between 1st January 2009 and 30th December 2013. A hundred and seventeen boys who underwent surgery for hydrocele due to patent processus vaginalis were reviewed. We determined their age at the time of surgery and the therapeutic aspects including the surgical approach, the practiced surgical gestures, post-surgery medical treatment and surgical treatment outcomes, including favorable cases and complications. Results: Mean age of the children was 44 months with extremes of 22 days and 15 years of age. Boys aged 3 to 11 were the most represented (58%); 38% were under 2. We performed a high ligation of the processus vaginalis in all children. Post-surgery medical treatment varied with a predominance of analgesics (98.3%) and antibiotics (73.5%). We recorded as post-surgery outcomes 112 favorable cases, one case of scrotal hematoma and four cases of recurrence. Conclusion: Many children with hydrocele had early surgical operation whereas they could have benefitted from surveillance of the patent processus vaginalis. Hydrocele recurrences are common in our study, this result probably related to the inexperience of the surgeons.

Le but de notre travail était de rapporter l’âge au moment de l’intervention et les aspects thérapeutiques chez les enfants opérés pour une hydrocèle à Dakar. Matériel et méthodes : Il s’agissait d’une étude rétrospective menée entre le 1er janvier 2009 et le 30 décembre 2013. Cent dix-sept garçons opérés pour une hydrocèle par persistance du canal péritonéo-vaginal ont été revus. Nous avons déterminé leur âge au moment de l’intervention et les aspects thérapeutiques incluant la voie d’abord chirurgical, les gestes pratiqués, le traitement médical post-opératoire et les résultats du traitement chirurgical incluant les cas favorables et les complications. Résultats : La moyenne d’âge des enfants était de 44 mois avec des extrêmes de 22 jours et 15 ans. La tranche d’âge de 3 à 11 ans était la plus représentée (58 %), 38 % des enfants avaient moins de 2 ans. Tous les enfants ont été opérés par une voie d’abord transversale au pli abdominal inférieur de canal péritonéo-vaginal était ligaturé à l’orifice inguinal profond et la collection liquidienne évacuée. Le traitement médical post-opératoire était variable avec une prédominance des antalgiques (98,3 %) et des antibiotiques (73,5 %). Dans les suites opératoires, 112 enfants ont évolué favorablement. Nous avons enregistré un cas d’hématome scrotal et 4 cas de récidives d’hydrocèle. Conclusion : Beaucoup d’enfants présentant une hydrocèle ont été opérés alors qu’ils pouvaient bénéficier d’une surveillance du fait des possibilités de fermeture spontanée du canal péritonéo-vaginal. Les récidives d’hydrocèle sont fréquentes dans la série, résultat probablement lié à l’inexpérience des opérateurs.
