Granulosa tumors of the ovary are among the rare ovarian tumors, and included in the sex cord-stromal tumor category. They have a low degree of malignancy and have a good prognosis. Their clinical presentation is sometimes suggestive. Some biological markers (serum inhibin B and AMH) may be helpful for diagnosis, though their sensitivity is not perfect. Preoperative imaging diagnosis remains challenging due to the wide variability in morphology and lack of epidemiological data in the literature. These tumors are accompanied by high levels of estrogen. This state of hyperestrogenism is responsible for a broad spectrum of endometrial pathology from simple endometrial hyperplasia to the development of adenocarcinoma of the endometrium. Through this clinical observation of tumor of the adult type granulosa, we will discuss the main clinicopathological characteristics and histogenesis of this association to improve treatment of these tumors.

Les tumeurs de la Granulosa de l’ovaire font partie des tumeurs rares de l’ovaire, du groupe des tumeurs des cordons sexuels et du stroma, hormono-sécrétantes. Elles ont un faible degré de malignité et sont de bon pronostic. Leur symptomatologie clinique est parfois évocatrice. Certains marqueurs biologiques (inhibine B et AMH) peuvent être informatifs pour le diagnostic mais ils ont une sensibilité encore imparfaite. Le diagnostic préopératoire en imagerie est un challenge, compte tenu de leur grande variabilité morphologique et du manque de données épidémiologiques et séméiologiques dans la littérature. Ces tumeurs s’accompagnent d’un taux élevé d’œstrogènes. Cet état d’hyperœstrogénie est responsable d’un large spectre de pathologie endométriale allant de la simple hyperplasie de l’endomètre jusqu’au développement d’un adénocarcinome de l’endomètre. À travers une observation clinique de tumeur de la granulosa de type adulte, nous discuterons les principales caractéristiques anatomo-cliniques ainsi que l’histogenèse de cette association afin d’améliorer la prise en charge de ces tumeurs
